Can you help me find a shorter document to print for her?

Some states do it over the phone, or will mail an application. Although many of the questions or pages don't apply in all circumstances, they have to be read and eliminated. States take phone applications. Or online applications. Or peruse it online to see the questions beforehand. Federal regs surely require a full financial snapshot.

4. According to AZ.gov, she can call and have one mailed to her or apply in person. No 30 pages of printing required.
