1. We need single payer now -

the executives can pay for extra care wherever in the world they seek it - but normal families can not afford this nonsense.

We need to tax high income individuals and particularly corporations and enact single payer by removing the age restrictions from Medicare or expanding Tri-care. We have a democracy here - the insurance companies would be free to offer supplemental care to folks for a fee above that. But basic coverage should be part of being an American. Everyone is issued a Social Security card and they could use that to check you in when you need care (it should come laminated with a small one you can put on your key chain). Basic preventative care and generic prescriptions - all covered. That in itself would offer a basic system that would at least allow folks to go to the doctor for annual physical, get their flu shot & other required vaccines, etc.

This really isn't so difficult.
